목적: 중등도 이상의 안구건조증 환자군에서 나노 에멀젼 방식의 0.05% 사이클로스포린 점안액과 양이온 에멀젼 방식의 0.1% 사이클로스포린 점안액을 사용하여 유효성 및 안전성을 비교하고자 하였다. 대상과 방법: 본 임상시험은 다기관, 이중맹검, 무작위 배정, 비교, 대조의 전향적 연구 방식을 채택하였다. 3개 기관 안과에서 중등도이상의 안구건조증으로 진료받는 환자 105명을 대상으로 시험군인 0.05% 사이클로스포린 나노 에멀젼 치료군과 대조군인 0.1% 사이클로스포린 양이온 에멀젼 치료군으로 무작위 배정하여 연구 시작 시점 및 안약 사용 1, 3개월째 약제의 유효성을 각결막염색점수, 눈물막 파괴시간, 쉬르머 검사를 통해 평가하였고, 약제 점안 시 불편감 및 순응도를 조사하여 안전성 결과를 비교하였다. 결과: 각결막염색점수, 눈물막 파괴시간, 쉬르머 결과상 두 군 모두 투약 초기 대비 유의한 개선 소견을 보였다. 각결막염색점수는두 군 모두 유의하게 감소하였으나 두 군에서 통계적인 차이는 없었다. TBUT는 투약 후 4주차에 시험군에서 유의한 연장 소견을보였고 쉬르머 검사에서는 12주차에서 시험군에서 유의하게 증가 소견을 확인하였다. 두 군 모두 치료 4주차에 비해 12주차에 약제사용시 안구 불편감 감소 및 순응도 증가를 보였으며 대조군이 12주차에 유의한 안구 불편감 감소를 보였다. 결론: 두 약제 모두 안구건조증 지표 향상 및 안구 불편감 경감에 효과를 보였으며, 사용 중 심각한 불편감이나 부작용은 관찰되지않아 약제의 유효성 및 안전성에서 유사한 결과를 보여주었다.
Abstract
Purpose: We evaluated the efficacy and safety of a 0.05% cyclosporine ophthalmic nanoemulsion and a 0.1% cyclosporine ophthalmic cationic emulsion in patients with moderate-to-severe dry-eye syndrome. Methods: This was a multi-center, double-blinded, comparative, prospective, randomized clinical trial. Three ophthalmological institutions enrolled a total of 105 patients with moderate-to-severe dry eye who were randomly assigned to a 0.05% cyclosporine nanoemulsion group (the test group) and a 0.1% cyclosporine cationic emulsion group (the control group). The corneal staining scores were obtained, and the tear film breakup time (TBUT) and Schirmer test performed, at the initial visit and after 1 and 3 months of eyedrop use. The safety results were compared by recording discomfort and compliance during drug instillation. Results: The corneal staining scores and the TBUT and Schirmer test results showed that both groups showed significant improvements. The corneal staining scores significantly (and similarly) decreased in both groups. In the test group, the TBUT showed significant prolongation after 1 month of treatment and the Schirmer test score increased significantly after 3 months. Both groups reported less ocular discomfort and better compliance with drug use at 3 months than 1 month; the control group demonstrated a significant decrease in ocular discomfort at 3 months. Conclusions: Both drugs improved the dry eye symptom index and relieved eye discomfort. No patient complained of serious discomfort or side-effects. The drugs were equivalent in terms of efficacy and safety.
