장딴지빗근 이는곳, 주행 및 닿는곳의 복합 변이: 증례보고

통상의 해부학 교육과정에서 만 89세의 한국인 남성의 종아리뒤칸 얕은근육무리에서 장딴지빗근의 이는곳, 힘살 주행 및 힘줄이 닿는곳에서 복합적인 변이가 관찰되어 다음과 같이 보고하고자 한다. 첫째, 장딴지빗근의 이는 곳이 넙다리뼈의 가쪽관절융기와 무릎관절주머니 위쪽의 오금면에서 시작하는 변이를 확인하였다. 둘째, 힘살의 주 행이 가쪽관절융기 안쪽과 정강신경의 사이를 따라 내려오는 변이를 관찰하였다. 마지막으로, 아주 짧은 형태의 힘줄 이 장딴지근의 가쪽갈래와 안쪽갈래가 만나는 지점에서 장딴지근 근막 앞면에 융합되는 변이를 확인하였다. 장딴지 빗근의 이러한 복합적인 변이는 우리가 알고 있는 한 이전에 보고된 바가 없는 경우로서, 장딴지빗근 힘줄의 수확 또 는 수술과 같은 외과적 접근이 필요한 의사들에게 임상적으로 유용한 해부학적 기초자료가 될 것이다.
Abstract
During routine educational dissection, we found complex variations of plantaris muscle, including origin, course of muscle belly, and tendon insertion, from the 89-years-old Korean male leg. First, we identified a variation in which plantaris muscle originates from the lateral condyle of the femur and the popliteal surface above the knee joint capsule. Second, variation was observed in the course of the muscle belly passing between the inside of the lateral condyle and the tibial nerve. Finally, the tendon was very short and showed an insertion variation that fused to anterior surface of the muscle fascia at the point where the lateral head and medial head of the gastrocnemius muscle merged. These complex variations of plantaris muscle have not been previously reported in our literatures review and will be clinically useful to surgeons for the surgical approach to harvest of tendon as well as for presence or absence of tendon.
