한국인 신경인지기능 평가를 위한 Cambridge Neuropsychological Test Automated Battery(CANTAB)의 신뢰성 평가

이번 연구는 국내 성인여성을 대상으로 CANTAB 신경인지기능 평가도구의 test-retest 신뢰성을 규명하는데 목적이 있다. 연구의 대상은 서울ㆍ경기 지역 중년여성 100명을 대상으로 하였으며, 실험에 참가한 모든 대상자들은 신경성질환이 없는 대상자들이 참여하였다. 연구의 측정 도구는 Cambridge Neuropsychological Test Automated Battery(CANTAB, UK)를 사용하였으며, 측정 항목으로는 Visual Memory Function 측정인 Delayed matching to sample(DMS), Paired Associates Learning(PAL)와 Executive Function 측정인 Spatial Working Memory(SWM), Spatial Span(SSP)로 총 4개 항목을 지정하였으며, 측정은 주위환경에 영향을 받지 않은 개인적 공간에서 연구자의 통제 아래 개별적으로 측정하였다. 본 연구의 결과, 첫째, 반복측정 t-test 결과는 SWM측정을 제외한 모든 변인에서 통계적인 차이를 나타내지 않았다. 둘째, 반복측정 결과 관련성은 모두 정적인 상관을 나타내었으며, DMS(r=.757, p<.01), SWM(r=.602,p<.01), PAL(r=.201, p<.05)순으로 통계적으로 유의한 관련성을 나타내었다. 셋째, bland & altman polt 결과 모든 변인의 차이 값의 평균이 1.5 미만으로 나타났으며, 95% 이상의 sample이 신뢰구간 안에 위치하였다; 따라서, 중년여성들에게 적용한 신경심리검사는 설문검사의 틀에서 벗어나 전산화된 검사를 이용하여 체육학 분야에서 활용적 가치가 높을 것으로 평가된다. 또한, 반복 측정 자료는 임상에서 매우 중요하게 활용되고 있어 검사도구의 신뢰성 확보가 중요하고 이번 연구를 통해 전산화된 신경심리 검사도구의 신뢰성을 확인하였다.
Abstract
The purpose of this study was to investigate reliability of CANNTAB from middle-aged women in Korea. Participate were involved one-hundred middle-aged women without nervous disease. Instrument of measurement was used Cambridge Neuropsychological Test Automated Battery(CANTAB, UK) and measurement factors were applied delayed matching to sample(DMS), paired associates learning(PAL), spatial working memory(SWM) and spatial span(SSP). Also, measurement was conducted in personal distance. Results are following: First, T-test of repeated measurement were not significantly difference in all factors expect SWM. Second, There were significantly correlation on DMS(.757**),SWM(.602**) and PAL(.201*). Third, Results of bland & altman plot showed that all factors of mean difference were lower than 1.5 and sample of more than 95% were confidence interval. Therefore,CANNTAB test is useful to evaluate a congnitive function in korean. Also, we investigated reliability of neuropsycical instrument, it can be used important clinical data.
