Abstract

Along with the continuously growing HIV/AIDS epidemic, there is a need for improvements in screening women for HIV infection. Diagnosing HIV infection is often a difficult task because of the lack of availability and affordability of laboratory testing. Research is needed that indicates the extent to which self-reported signs and symptoms are useful for screening women for HIV infection. One hundred and twenty five HIV negative (n=54; 43.2%) and HIV-1B positive (n=71; 56.8%) African- merican women ages 21 to 53 who participated in prospective cohort studies at the University of Miami's Comprehensive Drug Research Center were interviewed during 1993-1996 about signs and symptoms that may be associated with HIV-1B infection. Questions about symptoms were derived from the CDC's Multicenter Study of Crack Cocaine and HIV Infection, and from the Women's Interagency HIV Study. Classification tree analysis indicates that self-reported symptoms may be used to correctly classify 63.4% of HIV-1B positive women and 74.1% of HIV negative women, with an overall accuracy of 68.0%. Positive predictive value was 0.763, and negative predictive value was 0.606. Major physical symptoms and physical symptoms lasting more than thirty days are useful in the designation of women as HIV-1B positive. In conclusion, the refinement of protocols for assessing self-reported signs and symptoms of HIV infection and progression to AIDS is critical for the prevention, intervention, diagnosis, and treatment of HIV and AIDS among women.
