진행성 위암 환자에서 전이성 림프절로 오인된 육아종성 림프절병증 - 증례보고 -

영상 검사에서 림프절 종대 소견을 보이는 육아종성 림프절병증은 진행성 위암 환자에서 전이성 림프절 종 대로 오인함으로써 중요한 진단적 혼선을 야기할 수 있다. 우리는 영상학적으로 전이성 림프절이 의심되었 으나 조직검사를 통해 육아종성 양성 림프절병증을 진단한 3명의 진행성 위암 환자 증례들을 보고하고자 한다. 이 환자들은 컴퓨터 단층촬영 및 양전자 방출 단층촬영 스캔에서 악성 림프절병증을 시사하는 영상 학적 소견을 보였으나, 조직검사를 통해 양성 육아종성 림프절병증을 확진하였다. 첫 번째 환자는 결핵 (tuberculosis), 두 번째 환자는 사르코이드증(sarcoidosis), 세 번째 환자는 탄폐증(anthracosis)으로 진단되었 다. 이 증례들은 위암의 전형적인 전이 부위인 복강내 또는 좌측 쇄골 상부 림프절 외의 부위에서 림프절 종대를 보인 환자들로서, 비전형적인 전이 부위에서 림프절병증이 발생하는 경우 전이성 림프절 종대가 아 닌 다른 질환의 가능성을 반드시 고려하여야 한다. 이러한 경우 조직 생검을 통한 조직학적 진단이 적절한 치료를 시행하는데 필수적이다. 이 증례들은 진행성 위암 환자가 비전형적인 부위에 림프절병증을 보일 때 신중한 임상적, 영상학적, 조직학적 평가가 필요함을 강조한다.
Abstract
Granulomatous lymphadenopathy mimicking metastatic lymph nodes on imaging studies poses a significant diagnostic challenge in patients with advanced gastric cancer. We present three cases with advanced gastric cancer where radiological findings on computed tomography and positron emission tomography scans suggest malignant lymphadenopathy, but histopathological examination revealed benign granulomatous lymphadenopathy. The first case was diagnosed with tuberculosis, the second with sarcoidosis, and the third with anthracosis. These cases emphasize the importance of considering alternative diagnoses when lymphadenopathy occurs in atypical sites for gastric cancer metastasis, such as outside the intra-abdominal or left supraclavicular lymph nodes. In such cases, tissue biopsy is crucial to avoid misdiagnosis and ensure appropriate management. These cases highlight the need for careful clinical, radiological, and pathological assessments in patients with advanced gastric cancer presenting lymphadenopathy.
