Abstract

PurposeObesity is known to be a risk factor for a range of conditions in adults, including chronic diseases and cancer. Nonetheless, existing studies have presented inconsistent findings concerning the relationship between obesity and respiratory infections. This study aimed to examine the effect of childhood obesity on respiratory infections among children diagnosed with such illnesses. Material and methodsThe study analyzed data from 1,901,649 children aged between 3 and 5 years who received infant health screenings from 2015 to 2021. Based on the Korean National Growth Charts, children's body mass index (BMI) was categorized into normal and obese groups, and further classified into underweight, normal weight, overweight, and obese subgroups. Odds ratios(ORs) for respiratory infections were estimated using a multinomial logistic regression model, adjusting for sociodemographic variables and comorbidity scores. ResultsWithin one year following the health screening, 94.58% of all participants had received treatment for respiratory infections on at least two occasions. Multivariate analysis showed that childhood obesity was associated with a statistically significant increase in the risk of respiratory infections (OR 1.028, 95% confidence interval [CI]: 1.003–1.054). The risk of hospitalization due to respiratory infections was also higher among obese children (OR 1.029, 95% CI: 1.010–1.049). However, the observed effect sizes were small, indicating that the clinical significance may be limited. ConclusionThere was a statistically significant difference in the likelihood of outpatient visits and hospitalizations due to respiratory infections in children with obesity. This finding suggests the need for appropriate attention to the risk of respiratory infections in the health management of obese children.
