본 연구는 소아정형외과에 입원한 아동의 어머니가입원과 질환에 대해 경험하는 불확실성, 불확실성 평가및 대처의 영향요인을 파악하여 아동 어머니의 지지를위한 중재 전략을 수립하는데 기초자료로 활용하고자시도되었다. 자료수집은 2013년 1월 18일부터 9월 18 일까지 P 대학병원에서 IRB 승인을 받아 연구 참여에동의한 정형외과에 입원한 아동의 어머니 132명을 대상으로 이루어졌다. 대상자의 불확실성, 불확실성 평가에 대한 위험인지와 기회인지 및 대처 등을 측정하였으며, 도구의 신뢰도 Cronbach's alpha 값은 .76~.90 이었다. 대상자의 불확실성 정도는 평균 47.70점이었으며불확실성 평가의 위험인지는 평균 15.20점, 불확실성평가의 기회인지는 17.40점, 대처는 평균 141.77점이었다. 불확실성과 불확실성 평가는 정형외과 외 입원경험이 있는 경우에 따라 유의한 차이를 보였고, 대처는출생순위, 경제적 상태, 자녀수에 따라 유의한 차이를보였다. 불확실성은 불확실성 평가의 위험인지(r=.41, p<.001), 불확실성 평가의 기회인지(r=-.46, p<.001) 와 통계적으로 유의한 상관관계가 있었고, 불확실성 평가의 기회인지는 위험인지(r=-.43, p<.001)와 통계적으로 유의한 역상관관계가 있었으며, 대처는 불확실성 평가의 기회인지(r=.22, p=.011)와 유의한 상관관계가 있었다. 대상자의 대처에 영향을 미치는 요인은 자녀수, 불확실성 평가의 기회인지, 경제적 상태이었으며 이들의 설명력은 12%였다. 이상의 연구 결과를 통해 불확실성 평가의 기회인지가 정형외과 입원 아동 어머니의 대처에 중요한 변수로고려되어져야 함을 확인하였다. 소아정형외과에 입원한아동의 치료과정과 경과 등에 대해 충분히 설명하고 정확한 정보 제공과 상담을 통해 입원 아동 어머니의 불확실성에 대한 기회인지 평가를 증가시켜 대처를 증진시킬 수 있을 것이므로 이에 대한 중재가 마련되어야 한다. 또한 본 연구는 일 대학병원을 대상으로 하여 일반화에 신중을 기해야하며, 중증 질환은 제외되었고 급만성기 입원아동이 모두 포함되었으므로 추후 경과기간이나 중등도 및 다양한 질환을 대상으로 하는 반복연구를통해 비교연구가 필요하다.
Abstract
Purpose: The purpose of this study was to examine the uncertainty and coping of hospitalized children’s mother in an orthopedics ward. Methods: A total of 132 mothers of hospitalized children’s mother in an orthopedics ward participated in study. Data were collected from January 18, 2013 to September 18, 2013. The questionnaires measured the participants' responses according to the parents' perception of uncertainty scale, appraisal of uncertainty scale, coping health inventory of parents, and characteristics. Results: The uncertainty of hospitalized children's mother was related to hospitalization experiences except for orthopedic ward (F=-3.26, p=.001). The appraisal of danger by participants was also related to hospitalization experiences except for orthopedic ward (F=-2.39, p=.018). Coping was related to birth order (F=3.50, p=.033), perceived economic status (F=3.39, p=.037), and number of children (F=4.86, p=.009). Uncertainty was correlated with appraisal of danger (r=.41, p<.001), appraisal of opportunity (r=-.46, p<.001). Appraisal of opportunity was correlated with appraisal of danger (r=-.43, p<.001) and coping (r=.22, p=.011). In terms of the factors influencing coping, there were number of children (β=.22), appraisal of opportunity (β=.19), and perceived economic status (β=-.17). Conclusion: The results indicated that uncertainty correlated with appraisal of danger, appraisal of opportunity and coping. Factors influencing coping were number of children, appraisal of opportunity, and perceived economic status. Therefore, it is recommended the efforts to encourage mother’s appraisal of opportunity for improving their coping on their children’s hospitalization.
