본 연구는 신생아실과 산과병동 간호사의 대체수유에 관한 인지, 수행 및 교육요구를 측정하여 모유수유 실천을 돕는 간호중재 전략개발 기초자료를 제공하려는 목적으로 부산광역시내 7개 병원 간호사를 대상으로 시행되었다. 본 연구를 통해 신생아실간호사가 산과병동간호사보다 대체수유 인지와 수행이 많았고 간호사들의 대체수유 교육 요구가 높음을 확인하였다. 모유수유 실패의 많은 부분이 유두혼동으로 인한 것인데 반해 이를 보완해줄 대체수유 방법에 대해서는 제대로 알고 있는 간호사가 적고, 실제 병원에서 정확하게 수행하는 비율도 저조하여 본 연구에서 밝혀진바 적극적인 대체수유 방법의 교육과 수행이 필요함을 알 수 있다. 모유수유 권장은 임부의 산전관리 기간을 통하여 이루어져야 하며, 출산 후 이를 실천에 옮기기 위해서는 의료기관에서의 적극적인 모유수유 참여와 적어도 생후 첫 1개월간의 기간 동안 모유수유를 보호하는 간호사의 지속적인 지도나 상담이 필수요소라고 하겠다. 간호사는 모유수유증진을 위하여 매우 중요한 위치에 있고, 실제 모유수유 실천에서의 성공과 실패에 많은 영향을 주는 요인이 되며, 24시간 동안 모유수유에 영향을 줄 수 있는 중요한 위치에 있으므로 신생아 간호 인력의 대체수유에 대한 지식과 이들에 의해서 행해지는 정보를 확인해서 교정되거나 보충되어야 하는 어떤 부정확성이나 불충분한 정보를 확인하는 것과 수행정도를 확인하는 것이 필요하다. 본 연구는 대체수유에 대한 인지만을 조사하여 신생아실과 산과병동 간호사들의 대체수유에 대한 지식의 정도를 파악하는 것이 미흡했다. 반복연구에서는 대체수유지식의 정도를 측정할 수 있는 연구도구 개발을 제언한다. 또한 대체수유는 신생아에게 발생하는 건강문제 등과 관련하여 논란의 여지가 있으나 이를 명확하게 규명하고 대체수유의 효과를 검증할 수 있는 연구가 미흡하므로 대체수유 관련 유사실험연구를 제언한다. 더불어 표준화된 대체수유교육을 실시할 수 있는 프로그램개발에 관한 연구가 계속되어야 하며, 정규 교육과정에 표준화된 대체수유교육을 실시할 수 있도록 제도적인 뒷받침이 필요하다.
Abstract
Objectives:The purpose of this study was to identify whether alternative feeding performance and educational demands of nursery and OB (Obstetrics) ward nurses affects newborn feeding in preventing nipple confusion. Another aim of this research was to provide basic data to help in developing nursing intervention strategies and promote proper breast-feeding and alternative feeding performance. Methods:This descriptive research assessed and evaluated the alternative feeding performance and educational demands of nursery and OB ward nurses. Data analysis was conducted using SPSS Win 17.0 program and the measures used were t-test, ANOVA, and chi-square. The educational demands level was measured according to working departments using t-test. Results:The experience of performing alternative feeding was significantly different according to work place (χ2=25.54, p=<.001), work position (χ2=12.34, p=.002) and Rooming-in system (χ2=4.08, p=<.047). The knowing of alternative feeding according to work place (χ2=23.81, p=<.001). work position (χ2=11.66, p=.003) and Breast feeding experience (χ2=8.45, p=.037). The educational demands in alternative feeding showed significant difference according to Breast feeding experience in married pearson 82 people (χ2=8.04, p=.045). Conclusion:The success of promoting breast milk and alternative feeding techniques also depends on the medical team who looks after the mother and the newborn. In particular, nurses are responsible for supplying knowledge on proper breast feeding and alternative feeding and thereafter, assessing mothers' compliance. In effect, nurses should be provided with the necessary skills and knowledge to be able to play their roles in promoting proper breast milk and alternative feeding performance in actual settings.
