한국인 삼차신경통 환자에 대한 역학조사

삼차신경통은 남성에 비해 여성에서 더 흔하게 나타나며, 나이가 들어감에 따라 발병 위험이 커진다고 알려져 있지만, 이를 뒷받침하는 우리나라 삼차신경통 환자에 대한 역학조사는 드물거나 제한적이다. 또한 급속한 노령화로 인한 노인인구의 증가와 안전사고가 발생할 가능성이 높은 현대사회의 특성을 고려할 때, 삼차신경통의 발생 비율은 높아질 가능성이 매우 크다. 이 연구의 목적은 2010년부터 2017년까지 건강보험심사평가원에서 삼차신경통으로 분류된 환자들의 자료를 분석하여 연령별, 성별 유병률을 분석하고, 다른 나라의 연구결과와 비교하는 것이다. 우리나라 삼차신경통의 전체 평생 유병률은 0.15%이고, 남성보다 여성에서 유의하게 높았으며, 50세 이상 여성의 환자수는 2010년보다 2017년에 25% 이상 증가하였다. 연령 구간별 환자수 추이는 나이가 듦에 따라 점차 증가하는 경향을 보이며, 특히 50세 이후 급격한 증가를 보였다. 여성 55-59세 구간에서 가장 많은 환자수를 보이며, 75-79세 구간에서 가장 높은 유병률(0.273%)로 나타났다. 이러한 결과는 노령화가 가속되는 우리나라의 현 상황을 고려할 때, 삼차신경통에 의한 부작용과 사회, 경제적 부담이 커질 것을 대비하여 적절한 치료방법과 관련 보건의료정책이 조속히 이루어져야 함을 시사한다.
Abstract
It is well known that trigeminal neuralgia (TN) is more common in women than men and that the risk of onset increases with age. However, epidemiological studies for TN in Korea are rare or limited. In addition, considering the characteristics of modern society, which is characterized by an increase in the elderly population and the possibility of accidents, the incidence of TN is highly likely to increase. The purpose of this study is to analyze the prevalence of age and sex using the data of patients classified as TN at the National Health Insurance Review and Assessment Service (HIRA) of Korea from 2010 to 2017, and compare the results with those of other countries. The total lifetime prevalence of TN in Korea was 0.15%, which was significantly higher in women than men, and the number of women over 50 years of age increased by more than 25% in 2017 than in 2010. The number of patients grouped by age tended to increase gradually with age, especially showing a sharp increase after 50 years of age. The highest number of patients were found in the 55-59 years old group of women, and the highest prevalence rate (0.273%) was in the 75-79 years old group of women. These results suggest that appropriate treatment strategy and related health care policy should be promptly implemented in order to alleviate the side effects and socio-economic burden of TN.
