간호·간병통합서비스 병동 간호사의 공감력과 자기효능감이 노인 간호 수행에 미치는 영향

본 연구의 목적은 간호·간병통합서비스 병동에서 노인 환자를 간호하는 간호사를 대상으로 공감력, 자기효능감 및 노인 간호 수행 정도를 확인하고 노인 간호에 미치는 영향요인을 살펴보고자 하는 것이다. 이를 위해 S시 소재의 5개의 의료기관의 간호·간병통합서비스를 시행하는 병동에서 65세 이상의 노인 환자를 간호하는 210명의 간호사를 대상으로 자료수집을 실시하였다. 연구결과 대상자의 평균 연령은 29.9세, 간호·간병통합서비스 병동 근무 경력은 평균 1.92년으로 나타났다. 또한 공감력과 자기효능감(r=.33, p<.001), 공감력과 노인 간호 수행(r=.25,p<.001)에서 긍정적인 상관관계가 있는 것으로 확인되었다. 회귀분석 결과, 노인 간호 수행에 가장 큰 영향을 미치는 요인은 공감력(ß=.29,p<.001)으로, 간호·간병통합서비스 병동 간호사의 공감력이 높을수록 노인 간호 수행 정도가 높은 것으로 나타났다. 간호·간병통합서비스 병동 간호사는 노인 환자에 대해 다양한 요구에 민감하게 반응하며 이해하고 공감하려는 노력이 필요하다. 따라서 질적인 노인 간호를 제공하기 위해 노인에 대한 공감력을 증진시킬 수 있는 다양한 프로그램 개발이 요구된다.
Abstract
Purpose: The aim of this study was to investigate the influences of nurse's empathy and self-efficacy on nursing care of older adults in an integrated nursing care service (INCS) unit. Methods: The participants were 210 nurses caring for elderly patients in the INCS unit in Korea. Data were collected using self-report questionnaires from February 9 to February 23, 2017. The questionnairs are composed of empathy construct rating scale, self-efficacy scale and nursing care of older adults scale. Results: The mean age of the participants was 29.9 years old. There were significant positive relationships between age and clinical career (r=.78; p<.001), self-efficacy and empathy (r=.33; p<.001) and empathy and nursing care of older adults (r=.25; p<.001). The quality of nursing care of older adults were significantly different according to working experience in psychiatric unit (p=.021). Influencing factor of nursing care of older adults was empathy(ß=.29; p<.001), which explaned 30% of variance. Conclusion: The finding of this study indicated that attention should be given to empathy of nursing care of older adults and numerous efforts should be made to improve nurses' empathy for quality elderly care.
