의과대학 환자안전교육을 위한사례기반 학습모형 개발

초록

본 연구의 목적은 의과대학의 환자안전교육 맥락에서 사례기반학습이 효과적이고 효율적으로 실행되기 위한 학습모형을 개발하고 그 효과를 검증하는 데 있다. 2016년 전국 의과대학에 환자안전교육이 필수로 지정되면서 국내 의학교육에서 환자안전교육에 대한 수요와 관심이 증가하고 있으나 환자안전교육에 대한 인식 부족, 변화에 대한 강한 저항감 등으로 인하여 환자안전교육 방법에 대한 연구는 여전히 부족한 실정이다. 이에 본 연구는 설계․개발연구방법론을 활용해 의과대학 학생의사대상 환자안전교육의 최적화된 계획과 실행을 체계적이고 종합적으로 지원하는 학습모형과 실행지침을 개발하였다. 선행문헌고찰을 통해 국․내외 환자안전교육의 내용과 방법을 종합하여 초기학습모형 및 실행지침을 개발하고, 이후 4차에 걸친 전문가타당화를 통해 모형과 실행지침이 도출하였다. 외적타당화를 위해 의과대학 교수 1명과 본과 3학년 학생 29명 대상으로 본 모형 기반의 수업을 설계 후 실행하여 반응을 평가하고 그 결과를 반영한 형성적수정이 이루어졌다. 연구결과, 의과대학 환자안전교육을 위한 사례기반 학습모형과 17개의 실행 지침이 개발되었다. 학습모형은 1) 수업 목표 확인하기, 2) 사전지식 확인하기, 3) 학습조직 구성하기, 4) 사례 제시하기, 5) 사례분석 지원하기, 6) 환자안전요소 탐색 지원하기, 7) 사례기반 역할극 유도하기, 8) 환자안전 원칙개발 및 적용 촉진하기, 9) 성찰 유도하기의 아홉 단계로 구성되어 있으며, 실행지침은 각 학습 단계 별 하위 절차에 대한 예시 및 해설을 포함하는 형태로 개발하였다. 본 연구는 사례기반 환자안전교육을 위한 교수-학습요소를 종합적으로 탐색해 학생의사대상 환자안전교육을 종합적으로 지원하는 학습모형을 개발하였다는 점에서 의의가 있다.

Abstract

The purpose of this study is to develop a learning model for effective and efficient implementation of case-based learning for patient safety education in medical schools and to verify its effectiveness. As patient safety education was designated as mandatory in Korean medical schools nationwide in 2016, the demand and interest in patient safety education in domestic medical education is increasing. However the research on it is still lacking. Therefore, this study developed a learning model and implementation guidelines that systematically support the optimal planning and implementation of patient safety education for student doctors in the medical school by using the design and development research methodology. An initial model and implementation guidelines were developed by synthesizing the contents and methods of a review of the literature, and then the models and implementation guidelines were derived through expert validation over 4 rounds. For external validation, a medical school professor and 29 third-year medical school students participated. As a result of the study, a case-based learning model and 17 implementation guidelines were developed for patient safety education in the medical school. This study is meaningful in that it developed a learning model that comprehensively supports patient safety education for student doctors by exploring the teaching-learning elements for case-based patient safety education.
