근이영양증(근육병)환자 부모의 돌봄 경험

초록

본 연구는 근거이론접근방법을 이용한 내용분석을 통해 반성열성유전질환이면서 생명에 치명적인 장애를 주는 희귀질환인 근이영양증(muscular dystrophy) 환자 부모의 돌봄 경험을 탐구하여 그들의 경험을 이해하기 위한 질적연구(qualitative research)이다. 연구대상은 눈덩이표출방법을 적용하여 근이영양증 환자의 부모 15명(4명의 아버지와 11명의 어머니)을 선정하였는데, 환자는 모두 아들이었다. 연구에 참여한 근이영양증 환자의 부모로부터 면담한 자료와 관찰한 자료를 분석한 결과 중심현상은 ‘자식 살리기’로 나타났고, 이를 중심으로 한 근이영양증 환자 부모의 돌봄 경험 대한 패러다임 모형을 제시하면 다음과 같다. 자식 살리기의 인과적 조건은 ‘조기사망하는 근육병에 걸림’이었으며 맥락은 ‘모계유전질환’이었고, ‘가족갈등(병의 근원으로 인한 부부갈등; 치료의 기대차이에서 오는 부모-아들 갈등)’은 중심현상인 자식 살리기와 이를 위한 전략에 영향을 미치는 중재상황으로 나타났다. 자식을 살리기 위한 전략은 ‘부모가 합심하여 돌보기/어머니 홀로 돌보기’, ‘근육병환자 가족모임에 참여하기’, ‘좋다는 것을 다 해보기’, ‘운명으로 껴안고 아들의 원대로 해주기’였고, 결과는 ‘정 떼고 봉사하는 마음으로 돌봄’과 ‘죽은 자식 가슴에 묻음’으로 드러났다. 본 연구는 첫째, 한국의 사회문화에서 실존하는 근이영양증 환자를 돌보고 있는 부모를 대상으로 가족단위의 접근을 시도함으로서 근이영양증 환자의 가족체계, 가족기능, 가족원간의 갈등문제를 해결하기 위한 가족치료프로그램 개발에 기초가 되는 근거자료를 제공하였다는데 의의가 있다. 둘째, 다른 유전질환자 가족을 위한 가족기능강화서비스가 지속적으로 제공되어야 할 필요성에 대한 실증적 근거를 제공하였고, 셋째, 지역사회차원의 유전질환자 가족관리를 위한 가족보건복지차원의 중재를 위한 기초자료를 마련하였다는 데 의의가 있다. 본 연구에 따른 후속연구로서 근이영양증 환자의 부모를 위한 가족치료프로그램을 개발하여 그 효과를 확인하기 위한 사례연구와 근거이론접근법을 적용하여 근이영양증 환자와 환자가족의 유전질환에 적응하는 과정 및 적응유형을 파악할 수 있는 질적연구를 제언한다.

Abstract

Children with muscular dystrophy are unable to move on their own and need total care for any activities of daily living, and because most die before their parents, the parents experience serious crises. This study explored the experiences of 15 such parents. Data collection and analysis were guided by Strauss and Corbin's (1990) adaptation of grounded theory. Data were gathered via observation and in-depth semi-structured interviews. Results cover the range of parents' experience: ꡐSaving son's life from fatal diseaseꡑ emerged as the core phenomenon. 'Diagnosed as a disease of early death' was causal condition, and contextual condition was 'X-linked genetic disease'. 'Family conflict' was recognized as was recognized as intervening condition. The strategic action/reactions were 'caring of parents/caring of mother', 'participating muscular dystrophy association', 'doing everything to find a cure', and 'seeing events as destiny and doing as child pleases'. The consequences were 'caring for children with separation of parental affection' and 'burying their heart with their dead children'. The author recommends that these families be encouraged to access family therapy services for regulating family conflict and disease management.
