지역사회 통합돌봄을 위한 방문 재활 서비스가 재가 장애인의 삶의 질, 균형 능력, 인지 능력, 일상생활 활동, 작업 수행력에 미치는 영향

초록

연구목적: 본 연구는 지역사회 통합돌봄을 위한 방문 재활 서비스가 지역사회 장애인의 삶의 질, 균형 능력, 인지 능력, 일상생활 활동, 작업 수행력에 미치는 영향을 확인하기 위해서 수행되었다. 연구방법: 총 16명의 대상자가 연구에 참여하였고, 인지 장애 8명, 자폐스펙트럼 장애 2명, 뇌병변 6명이 포함되었다. 재활 서비스는 1) 언어 재활, 2) 미술 치료, 3) 인지 재활, 4) 심리 운동, 5) 감각 재활, 6) 운동 재활 6가지로 구분되었고, 각 회당 50분씩 주당 2회 8주 동안 제공되었다. 중재 전후로 삶의 질, 균형 능력, 인지 능력, 일상생활 활동, 작업 수행력을 평가하였고, 추가적으로 지적장애, 지체장애, 자폐성 장애의 질환에 따른 중재효과를 비교하였다. 정규성 검정을 만족하는 경우 모수통계를 사용하였고, 대상자의 숫자가 적거나 정규성을 만족하지 못하는 경우에는 비모수 통계를 사용하였다. 연구결과: 서비스 제공 전, 후로 COPM 수행도, COPM 만족도, FIM 사회인지, FIM 총계, WHOQOL 총점, ABC 점수는 통계적으로 유의한 차이가 관찰되었다(p<.05). 하지만 FIM Sphincter Control, FIM Mobility, FIM Communication, K-MOCA에서는 유의한 차이가 관찰되지 않았다. 질환에 따른 집단 간 비교에서는 삶의 질의 사회적 인지와 균형 능력에서 통계적으로 유의한 차이가 발견되었다. 사후분석의 결과 지적장애 집단과 지체장애 집단에서 통계적으로 유의한 차이가 발견되었다. 결론: 지역사회 통합돌봄을 위한 방문 재활 서비스는 재가 장애인의 삶에 긍정적인 영향을 미치고, 사회적 인지및 균형 능력의 경우 지적장애 집단에서 뇌병변 장애 집단보다 더 큰 효과가 관찰되었다. 본 연구의 결과는 지역사회 장애인을 위한 통합돌봄 서비스의 근거자료로 활용될 수 있을 것이다.

Abstract

Purpose: This study aimed to investigate the impact of rehabilitation services provided by community-integrated care resources on the quality of life, balance control, cognitive function, activities of daily living, and work performance of individuals with disabilities who receive home-based care. Method: Sixteen participants were enrolled and were classified as having an intellectual disability (n= 8), autism spectrum disorder (n= 2), or a brain lesion (n= 6). Six types of rehabilitation services were offered, including speech rehabilitation, art psychology, cognitive rehabilitation, psychological exercise, sensory rehabilitation, and motor rehabilitation. The study evaluated the participants' quality of life, balance, cognitive function, activities of daily living, and work performance before and after each service was provided. Results: COPM performance, COPM satisfaction, FIM Social Cognition, FIM Total, WHOQOL Total, and ABC Total scores obtained before and after participants received services were statistically significant (p<.05). However, there were no significant differences observed for FIM Sphincter Control, FIM Mobility, FIM Communication, and K-MOCA. In the classification according to the diagnosed type, statistically significant differences between groups were found in FIM social cognition and ABC total (p<.05). Conclusion: Home-visit rehabilitation services provided by community-integrated care facilities had a positive impact on the activities of daily living, quality of life, occupational performance, and balance of individuals with disabilities who received home-based care. The findings of this study provide valuable evidence for community-integrated care projects. Therefore, the results of this study provide relevant evidence for community-integrated care projects.
