지역사회에서 서비스제공자와 가족으로부터 경험하는 정신장애인의 지각된 강요에 관한 연구

본 연구는 입원 못지않게 지역사회 서비스이용과정에서도 정신장애인이 강요를 지각할 수 있다는 전제를 가지고 그들이 가족과 서비스제공자로부터 지각하는 강요의 예측요인과 그것이 삶의 만족에 미치는 영향을 분석하였다. 이를 위해 지역사회 정신건강서비스를 이용하는 정신장애인 170명을 대상으로 가족으로부터, 서비스제공자로부터의 지각된 강요와 인구통계학적 변인, 병식, 증상, 사회적 기능, 진단명과 같은 임상적 변인 그리고 그들의 삶의 만족을 조사하였다. 그 결과 서비스제공자로 인한 지각된 강요가 가족으로 인한 지각된 강요보다 높았고 이 둘 간에는 의미 있는 상관관계가 있어 한 유형의 강요를 높게 지각하면 다른 유형의 강요도 높게 지각하였다. 두 유형의 지각된 강요에 대한 인구통계학적 변인의 예측력은 통계적으로 유의하지 않았으나 임상적 변인 중 증상이 두 유형 모두에 의미 있는 예측력을 보였다. 그 외의 임상적 변인 중 사회적 기능은 서비스제공자로부터의 지각된 강요를, 진단명과 병식은 가족에 의한 지각된 강요를 의미 있게 예측하였다. 또한 임상적 변인을 통제한 상태에서 두 유형의 지각된 강요가 삶의 만족에 미치는 영향력을 분석한 결과 의미 있는 영향력을 보이지 않았다. 따라서 삶의 만족은 지각된 강요가 아닌 임상적 변인이 더 의미 있는 설명력을 가지는 것을 알 수 있다. 이러한 결과를 바탕으로 연구자들은 최소한의 규제를 표방한 지역사회에서 지각된 강요를 감소시킬 수 있는 임상적 개입, 역량강화 전략 등을 제안하였다.

Abstract

This study aims to analyze the effect of socio-demographic and clinical variables of people with mental illness on two types of perceived coercion, that from service providers and family members. Subsequently, when controlling clinical variables, we also analyze the effect of these perceived coercion types on life satisfaction. We evaluated several clinical variables(symptoms, psychosocial functioning, diagnosis, and insight), levels of perceived coercion from service providers and family, and life satisfaction in 170 people with mental illness living within the community and useing ae community-based mental health program. The results, indicate that perceived coercion from service providers was higher than that from family. Some clinical variables had a significant effect on both types of perceived coercion, but demographic variables had no significant effects. When controlling clinical variables, neither perceived coercion type had a significant effect on life satisfaction. Based on these findings, the researchers suggest a strategy to reduce coercion that supports minimal restrictiveness and clinical controls.
