본 연구는 일개 병원 권역응급의료센터에 내원한 자전거 손상 환자들을 연도별로 조사하여 손상의 임상적 특징과 사망원인을 분석하기 위한 연구이다. 본 연구는 교통사고 심층 손상자료를 이용해 2006년 8월부터 2013년 12월까지의 자전거 손상 환자를 대상으로 후향적 연구를 시행하였다. 자전거 손상의 연도별 임상 특징은 사지 및 골반, 두경부에서 손상이 많았고, 자전거 손상의 사망원인은 두부 손상이 가장 많았다. 타 연구에서도 자전거 사망은 대부분 두부 손상에 의한 것이었다. 자전거 손상과 사망을 줄일 수 있는 헬멧의 착용률은 현저히 낮았으며 두부 손상으로 사망한 사람은 헬멧을 착용하지 않았다. 연구 결과를 토대로 자전거 손상을 예방할 수 있는 정책 마련과 일반적 환자처치에 기초자료를 제공할 수 있다. 그리고 자전거 손상사고가 연도별로 꾸준한 비율을 보이고 있으므로 손상감시의 심층조사가 필요하다.
Abstract
This study examined the clinical characteristics and causes of death among bicycle injury patients who visited an emergency room. From August 2006 to December 2013, we collected data on the injured patients including age, sex, AIS, ISS, death rate, and so on. Among 1.023 patients treated in an emergency room during the period, 341 (33.3%) were hospitalized, 47(4.6%) received an emergency surgery, and 23 (2.2%) died. It is also found that only 7.1% of those patients wore helmet. Most of the patients had injuries on limbs, pelvis, and head and neck, and the death rate among patients with head and neck injuries appeared to be high. The findings of this study highlight a need of a better understanding on the severity of bicycle injuries and suggest active promotion of wearing protecting equipment along with policy establishment.
