경찰 응급처치 교육의 패러다임 변화 필요성에 대한 연구

병원 전 단계에서의 응급처치 실시 여부는 생명 유지에 직접적인 영향을 미칠 만큼 매우 중요하고, 구급대원이 도착하기 전 현장 경찰관에 의한 응급처치 역시 최초반응자로서 간과해서는 안 되는 부분이다. 현장대응능력 향상을 위해 경찰의 응급처치 교육은 기존 심폐소생술 중심의 교육 전개에서 벗어나 심각한 출혈 처치 등 현장의 특성을 적극 반영해야 하며 보다 현실적인 응급처치 교육을 실시할 필요가 있다. 이에 본 연구를 통해 국내외에서 시행 중인 응급처치 교육의 동향을 살펴보고 교육의 패러다임 변화 필요성을 강조하고자 한다. 미국, 유럽, 일본 등에서는 응급처치 가이드라인을 기반으로 표준화된 교육이 실시되고 있었는데 변화된 응급처치 교육의 가장 큰 특징은 심각한 출혈에 대한 처치였다. 심각한 출혈이 예방 가능한 사망 원인으로 인식됨에 따라 최초반응자에 의한 지혈대, 지혈 거즈 등의 적극 사용으로 이어졌다. 美 경찰 또한 관련 교육 실시와 함께 개인 응급처치 키트를 보급하여 경찰 현장에서 적극 활용 중에 있었다. 최근 우리나라 軍 응급처치 교육의 변화는 이러한 흐름을 잘 나타내 주고 경찰에 시사하는 바가 크다. 응급처치 가이드라인 마련, 표준화된 교육 체계 구축, 개인 장비 도입은 교육 패러다임 변화의 중요한 과제라 할 수 있다.
Abstract
Whether or not first aid is provided in the pre-hospital phase is very important to have a direct impact on life maintenance, and first-aid measures by on-site police officers before paramedics arrive are also a part that should not be overlooked as a first responder. In order to improve response capabilities, police first aid training should depart from the existing CPR-centered training and actively reflect the characteristics of the field, such as treatment of severe bleeding, and it is necessary to implement more realistic first aid training. Therefore, this study aims to examine the trends in first aid training in Korea and abroad, and to emphasize the need for a paradigm shift in police first aid training. In the United States, Europe, and Japan, standardized training based on first aid guidelines was being conducted. Serious bleeding was recognized as a preventable cause of death, leading to active use of tourniquets and hemostatic gauze by first responder. The U.S. police also provided personal first aid kits along with relevant training and were actively using them at the police scene. The recent change in first aid education in the Korean military reflects this trend well and has great implications for the police. The preparation of first aid guidelines, the establishment of a standardized training system, and the introduction of personal equipment are important tasks in the paradigm shift in police first aid training.
