본 연구에서는 세대간 회계를 이용하여 건강보험 재정부담의 세대별 귀착에 대한 분석을 시도하였다. 현행의 건강보험제도와 조세제도를 유지할 경우, 향후 인구 의 노령화와 소득상승에 따른 의료수요 증대와 세입기반의 약화로 인해 향후 건 강보험 재정수지가 악화되어 미래세대의 재정부담이 급격히 증가할 것으로 예상 되며, 이러한 현상은 건강보험료와 국고지원금의 상대적 비중 등 건강보험 세입 구조와 재정운용방식에 상관없이 발생할 것으로 보인다. 또한 2000년 7월부터 실 시되고 있는 의약분업과 건강보험 재정통합은 건강보험 재정부담을 전반적으로 증가시킨 것으로 나타났다. 마지막으로 의료비 지출 억제를 위한 수요측면과 공 급측면에서의 유인구조 강화, 그리고 건강보험의 보험적 기능 강화를 위한 포괄 수가제도와 같은 사전적 지불보상제도와 경증진료에 대한 급여축소, 중증진료에 대한 급여확대의 필요성과 실현가능성도 확인할 수 있었다.
Abstract
We use Generational Accounting to study the incidence of the fiscal burden of National Health Insurance (NHI) in Korea. We show that the current NHI system is not financially sustainable, because of prospective large-scale benefit expenditure increase and limited tax base due to population aging and increasing demand for public expenditure for health. This result holds regardless of the revenue structure of the NHI, such as revenue proportion of contribution and government subsidy, and budget management method. We also show that July 2000 reforms (financial integration and separation of prescribing and dispensing drugs) substantially increased the fiscal burden of the NHI. Finally, we confirm the necessity and implementability of prospective payment method, such as Diagnosis Related Group system (DRG), and reinforcement of risk pooling function of the NHI, by increasing service fees for severe diseases accompanied with decrease of fees for minor ailments.
