Abstract
Strengthening the public sector in health care delivery has been actively discussed recently. Increasing the number of public hospitals has been the major focus because the public sector accounts for less than 10% of all hospitals. However, rather than increasing the number of public hospitals, policy priority should be given first to reinforcing the public mission and strengthening the programs of the existing public hospitals. It is of critical importance to identify and implement policy measures to remove managerial and environmental barriers to the efficient operation of public hospitals and their contribution to the equity in the health sector. Government also needs to encourage not-for-profit hospitals to provide community or public benefits. Since the out-of-pocket payment by the patient accounts for more than 50% of national health care expenditure, strengthening the role of the public sector in health care financing can be more efficient and equitable than increasing the role of public sector in health care delivery or increasing the number of public hospitals.
