본 논문의 목적은 통합 의료적 접근이 어떻게 지역사회 참여와 공동체 역량 강화라는 순기능으로 이어지며 건강의 형평성을 보장하는지 쿠바 사례를 중심으로 접근하는 것이다. 통합의료적 접근은 개별 질병 치료 중심의 임상의학과 구별되는 것으로, 본 연구에서는 쿠바 보건체계의 주요 토대인 일차의료에 기반을 두는 커뮤니티 케어를 통합의료의 조직적 관점과 내용적 측면의 두 수준으로 나누어 분석하였다. 진료소와 폴리클리닉을 중심으로 이루어지는 지역공동체 기반의 일차보건의료체계를 조직적 측면에서 살펴보았고, 공동체 내 다 부문 간 의료적 실무의 유기적 협력이 이루어지는 양상을 통해 통합의료의 내용적 측면을 살펴보았다. 이를 통해, 본 연구에서는 쿠바 보건체계의 통합 의료적 성격이 어떻게 지역사회 내 건강 불평등을 최소화하면서 지역주민의 높은 건강 수준을 유지할 수 있는 토대임을 확인하고자 하였다.
Abstract
The purpose of this paper is to analyze the Cuban Health System as Integrated Care focused on Community Care. The integrated care is a distinct approach from clinical medicine, which is aimed at treating disease, leading to the net function of community participation and community empowerment in order to guarantee the equity in heath. In this paper, the Cuban health system will be examined into two perspectives, which is the organization level where the primary health care system centered on clinics and polyclinics, and the health care program based on organic coordination between multidisciplinary sectors within the community. Upon this study, it is expected to explain how the Cuban health system continues to maintain a high level of population health while minimizing health inequality in the community as well.
