초록
Purpose : This study aimed to identify factors that influence the transition to insulin treatment in patients with type 2 diabetes. Methods : The final study subjects consisted of 152 patients with type 2 diabetes who were attending the endocrinology department of a general hospital in D City. The study examined the stage of change in insulin treatment, 10 change processes, decisional balance(pros and cons of insulin treatment), self-efficacy through a survey. Results : The results of the univariate analysis revealed significant differences in 8 change processes (dramatic relief, self reevaluation, social reevaluation, social liberation, self liberation, counter conditioning, stimulus control, and reinforcement management), as well as decisional balance(pros and cons of insulin treatment) and self-efficacy depending on the stage of insulin treatment change. Further multinomial logistic regression analysis showed that self reevaluation(OR=1.487, p=.010) during the pre-contemplation stage and social reevaluation(OR=1.342, p=.025) during the contemplation stage had significant effects on the transition between stages. Additionally, it was confirmed that barriers related to insulin treatment were found to be the main factor preventing patients with type 2 diabetes from initiating insulin treatment. Conclusions : Based on these findings, when designing interventions to encourage insulin treatment implementation, it is recommended to enhance the processes of change including self reevaluation and social reevaluation considering the patient’s stage of change, and to use strategies aimed at reducing barriers.
