알레르기성 진균성 부비동염(allergic fungal sinusitis, AFS)은 진균성 부비동염의 비침습적인 형태로서 IgE 매개성 과민반응에 의해 발병한다. AFS를 진단하기 위해서는 type I 과민반응을 증명할 수 있어야 하고, 특징적인 알레르기성 점액과점액 내 진균을 확인할 수 있어야 하며, 그 외에도 다발성 비용 및 특징적 컴퓨터 전산화단층촬영 소견이 있어야 한다. 발생빈도는 지역적인 차이가 있으며 국외에서는 많은 보고가 있으나 국내 보고는 미미한 상태이다. 저자들은 전형적인AFS 환자에서 진균 배양 검사상 Curvularia species가 동정되어 국내 첫 증례로 보고하는 바이다. Abstract
Allergic fungal sinusitis (AFS) is a noninvasive form of fungal rhinosinusitis resulting from an IgE-mediated hypersensitivity reaction. The diagnosis of AFS can be established by demonstrating type I hypersensitivity,presence of fungus in mucus, eosinophilic mucin, nasal polyposis, and characteristic CT scans. Although AFS is not unusual and its incidence may be increasing, few cases have been reported in Korea. Here, we report the first case of typical AFS in which Curvularia species was isolated by culture. (Korean J Clin Microbiol 2010;13:98-101)
