상악 완전무치악에서 지르코니아 framework을 이용한 임플란트 지지형 고정성 보철 수복 증례

우식 및 치주질환등의 여러원인으로 다수의 치아를 발거 후 안면 및 치조골의 해부학적인 변화가 일어난다. 상악의 경우 치조골이 치근의 방향과 경사로 흡수되고 잔존치조골이 짧아지면서 악궁의 직경이 감소하게 된다. 또한 비순각(Nasolabial angle)이 커지면서 입술의 지지를 상실하게 되어 안면의 심미성을 상실하게 된다. 이를 재건하기 위해 다양한 수복 치료가 제안될 수 있다. 무치악 환자에게 보철적 재건 방법 중 전통적인 총의치 치료가 선택될 수 있지만 환자의 대부분은 의치에 의한 통증과 불편감, 안정성 부족으로 자연치를 갖는 사람들보다 저작능력이 20%미만이다. 또한 의치가 신체의 일부가 아님을 인지하고 심리적으로 만족하지 못하는 경우가 종종 있으며 이를 위해 저작기능 향상과 유지, 안정 그리고 치조골을 보존 할 수 있는 임플란트를 이용한 가철성과 고정성 보철 치료가 다른 대안이 될 수 있다. 상악 무치악 부위를 위한 임플란트 치료방법 선택 시 심미성을 위해 보철물의 변연을 원치 않거나 적절한 입술 지지, 발음의 향상, 구개를 덮지 않아 환자에게 편안함을 주는 방법으로 임플란트를 이용한 fixed detachable prosthesis 치료를 선택할 수 있다. 본 증례는 상악 잔존치조골 흡수로 부적절한 악간관계와 입술지지를 잃은 무치악 환자를 대상으로 지르코니아 framework을 이용한 임플란트 지지형 고정성 보철 치료를 진행하였고 기능적, 심미적으로 만족할 만한 결과를 얻었기에 이를 보고 하고자 한다.
Abstract
Anatomical changes in the facial and alveolar bones occur after multiple teeth are extracted. In the maxilla, the alveolar bone is absorbed in the direction and inclination of the root, and the remaining alveolar bone becomes shorter, reducing the diameter of the arch. In addition, as the nasolabial angle increases, the support of the lips and the aesthetics of the face are lost. This case reports a functional and aesthetically satisfactory results of full mouth rehabilitation with the implant-supported fixed prosthesis using a zirconia framework.
