임플란트 주위염이 발생한 고정성보철 환자에 상악 Milled-bar와 부착장치를 이용한 피개의치로 수복한 증례

완전 무치악 환자의 보철 수복에는 다양한 방법이 있지만, 환자의 개별적인 상황과 요구사항에 따라 적절한 방법을 선택하는 것이 중요하다. 가철성 총의치는 비교적 경제적인 장점이 있지만, 불편한 착용감과 낮은 저작효율이 문제가 될 수 있다. 반면 임플란트를 이용한 고정성 보철물은 안정성이 높지만 다수의 임플란트가 필요하고 수술적인 위험이 있으며 경제적인 부담이 크다는 단점이 있다. 임플란트 보철학이 도입되면서 임플란트를 이용한 임플란트 지지 국소의치, 피개의치 등 다양한 선택이 가능해졌다. 잔존 치조골이 충분한 경우 임플란트를 이용한 고정성 보철물이 가장 효과적인 선택일 수 있다. 하지만 잔존 치조골이 부족하여 임플란트 식립이 불가능하거나 경제적인 여유가 없는 경우에 소수의 임플란트를 이용한 피개의치는 임플란트를 통해 지지를 얻고 부가적인 부착장치로 유지를 얻을 수 있으며 의치로 안모를 회복하는데 적합할 수 있다. 본 증례는 하악 양쪽 구치부 임플란트 통증 및 저작시 불편감으로 내원하였으며, 심한 임플란트 주위염으로 다수의 임플란트를 발거 후 상악은 양쪽 구치부에 임플란트를 재식립하여 milled-bar에 부착장치로 유지력을 얻은 피개의치를 제작하였고, 하악은 임플란트를 추가 식립 후 고정성 보철물을 이용하여 수복하였다. 치료가 완료된 후 10개월간 경과관찰 결과 의치의 파절, 유지력 상실 등 임상적으로 이상이 없었으며 방사선학적으로도 초진시와 유사한 골 소실상태를 보였다. 또한 구강위생이 잘 유지되었고 환자도 심미적 기능적 결과에 만족하였다.
Abstract
There are various methods for restoring the dentition of completely edentulous patients. Removable complete dentures have the advantage of being relatively economical, but they can be uncomfortable to wear. With the introduction of implant prosthodontics, various options such as implant-supported overdentures and hybrid prostheses have become available. If there is inadequate remaining ridge or limited financial resources, an overdenture supported by a few implants with additional attachments may be more suitable. In this case, due to severe peri-implantitis and other complications, the implants were removed. Subsequently, four implants (two on each side) were placed in the maxilla and a milled-bar with attachment was fabricated for each side of the maxilla.
