다수치아 발치 후 소수 잔존치를 가진 부분 무치악 환자의 보철치료에는 임플란트 고정성 보철물, 전통적인 국소의치가 있다. 임플란트 고정성 보철은 저작효율 및 만족감은 좋지만 다수의 임플란트 식립이 필요하므로 수술 및 비용적인 부담이 있다. 소수 치아만 남게 된 상태에서 후방연장 국소의치 제작 시에는 치아에서 얻을 수 있는 지지가 작아져 조직의 부담이 커진다. 이는 의치의 안정성과 지대치의 수명을 위협한다. 최근에는 이를 극복하기 위해 소수의 임플란트를 보조적으로 사용하여 부가적인 유지, 지지를 얻을 수 있는 임플란트 보조 국소의치(IARPD; Implant-assisted removable partial denture)가 하나의 치료방법으로 떠오르고 있다. 본 증례는 다수치 발거 후 소수 잔존치만 남아있는 하악 부분 무치악 환자에서 임플란트 크라운을 활용한 임플란트 보조 국소의치를 제작하여 대칭성을 확보하였고, 환자 및 술자 모두 심미, 기능, 경제적으로 만족스러운 결과를 보였기에 보고하는 바이다.
Abstract
Implant-supported fixed prostheses and conventional removable partial dentures are standard treatment options for partially edentulous patients with only a few remaining teeth. While implant-supported fixed prostheses provide excellent mastication efficiency and patient satisfaction, they require multiple implant placements, posing both surgical and financial burdens. In cases where only a few teeth remain, fabricating a distal extension removable partial denture results in limited support from the remaining teeth, increasing the load on the surrounding tissues. This compromises the stability of the denture and reduces the longevity of the abutment teeth. Recently, implant-assisted removable partial dentures (IARPDs) have emerged as a viable treatment option, offering additional retention and support through strategically placed implants. This case report presents the fabrication of an IARPD using implant crowns for a partially edentulous mandible with only a few remaining teeth after multiple extractions. The treatment achieved satisfactory outcomes in terms of aesthetics, function, and cost-effectiveness for both the patient and the clinician.
