임신과 연관된 골다공증은 매운 드물다. 최근까지 임신성 골다공증의 병태생리는 불분명하고 치료 방법 또한 잘 알려져 있지 않다. 임신성 골다공증은 지속적인 심한 요통과 신장 감소 및 하나 이상의 척추 골절을 동반한다. 대부분의 경우 임신 3분기 또는 분만 직후의 초산부에서 흔히 나타난다. 본 증례는 26세 초산부 심한 임신성 골다공증에 대한 증례로 임신 10개월에 중등도의 요통을 호소한 후 분만 직후 요통이 악화된 예이다. 골밀도 검사상 요추(1-2)의 T 수치는 -3.5이며 총 대퇴골 T 수치는 -1.0이다. 임상 병리 검사상 이상 소견은 없었다. 임신성 골다공증이 드문 질환이나 통증이 임신 3분기 또는 분만 직후에 발생한다면 감별진단으로 고려해야 한다.
Abstract
Osteoporosis associated with pregnancy is a rare condition. To date the pathophysiology of this entity of bone disorder is still uncertain, and its therapeutical management is poorly defined. It causes one or more vertebral fractures with severe, prolonged back pain and height loss in affected women. Majority of the cases are seen in the third trimester or just after delivery in primagravid women. In this case report, a 26-year-old woman who had severe pregnancy-induced osteoporosis was presented. During last month of her first pregnancy she had moderate back pain. After delivery, the back pain has gotten worse. In the bone mineral density, L(1-2) T score was -3.5 and total femoral T score was -1.0. There was no abnormality in the laboratory findings. Although pregnancy-associated osteoporosis is a rare condition, when pain occurs in the last trimester or early postpartum period, it should be considered in differential diagnosis.
