고령장애인의 우울, 신체적 기능, 그리고 사회적 유대감이 자살위험에 미치는 영향: 삼원 상호작용 효과를 중심으로

초록

본 연구의 목적은 고령장애인의 자살위험에 영향을 미치는 주요 변수를 신체, 정신, 그리고 사회건강적 관점에 따라 우울, 신체적 기능, 사회적 유대감으로 구분하고 자살위험과의 관계를 검정하여 정책 및 실천적 개입의 단초를 제공하는데 있다. 이를 위해 2021년 경기도지체장애인협회에서 고령장애인 쉼터 이용자 대상 실태조사에 응한 총 202명에 대한 데이터를 활용하여 분석을 실시하였다. 연구의 주요결과를 요약하면 다음과 같다. 첫째, 고령장애인 자살위험군일수록 정신 및 사회건강이 유의미하게 부정적인 양상을 보였으나 인구사회학적인 요인과 신체건강 상의 차이는 유의미하지 않은 것으로 나타났다. 둘째, 고령장애인의 우울, 신체적 기능, 그리고 사회적 유대감의 삼원 상호작용항은 자살위험을 부적으로 예측하였다. 이는 우울 정도가 심각한 고령장애인일수록 신체건강과 사회건강의 영향을 탄력적으로 받아 자살위험의 영향력이 달라지는 것을 의미하는 것으로서, 고령장애인의 자살위험 완화를 위해서는 다체계적 개입이 전제될 필요가 있음을 의미한다. 이러한 연구결과를 토대로 정신, 신체, 사회건강 증진을 위한 정책 및 실천적 개입방안을 제언하였다.

Abstract

The purpose of this study is to categorize the main variables associated with the suicide risk of older adults with disabilities into depression, physical functioning, and social bonding according to physical, mental, and social health perspectives, and to test their relationship with suicide risk to provide a basis for policy and practical interventions. We used 202 data of older adults with disabilities who participated in surveys on the satisfaction with use of facilities for persons with disability conducted by Korea Association of Persons with Physical Disabilities in 2021. The main results are as follows. First, depression and physical functioning were significantly more negative among those at risk of suicide, however, no differences found in demographic factors and physical health. Second, the three-way interaction of depression, physical functioning, and social connectedness was negatively predictive of suicide risk. This suggests that the risk of suicide is influenced by the elasticity of physical and social health among older adults with severe depression, suggesting that multisystemic interventions are necessary to reduce the risk of suicide among older adults with disabilities. Based on these findings, we recommend policy and practical interventions to promote mental, physical, and social health.
