혈액투석(Hemodialysis)을 받는 신장장애인의 우울 경험에 관한 연구 : 근거이론을 적용한 질적 연구

본 연구는 혈액투석을 받는 신장장애인들의 우울 경험과 그 과정에 대한 심층 분석을 목적으로 한다. 이를 위해 주관적으로 우울을 경험한 혈액투석 중인 신장장애인 20명을 심층 면담하였으며, Strauss와 Cobin(2007)의 근거이론 방법론을 활용하여 분석하였다. 주요 분석결과는 다음과 같다. 첫째, 개방코딩 결과, 128개의 개념과 45개의 하위범주, 17개의 상위범주가 도출되었다. 둘째, 축코딩 패러다임 결과, 인과적 조건은 ‘불가항력 적인 투석’, ‘벗어날 수 없는 고통스러운 투석의 삶으로 추락’, 중심현상은 ‘무너져버린 나의 삶’, 맥락적 조건은 ‘신장 장애에 대한 사회적 인식 부족’, ‘내 삶을 묶어놓은 만성장애’, ‘나를 더 억누르는 경제적 어려움’, ‘투석으로 인한 신체적 고통’, ‘기약 없는 뇌사자 기다림’, 중재적 조건은 ‘나를 둘러싼 사회적 지지체계’, ‘신장장애인 간의 정보교류와 정신적 지지’, ‘신장장애인의 우울 완화를 돕는 정책 및 서비스’, 작용·상호작용 전략은 ‘나를 만들어가는 긍정적인 태도’, ‘새로운 사회적 지지체계 형성하기’, ‘이타적 행동 속에 얻는 긍정적인 힘’, 결과는 ‘마지못해 살아가는 나의 삶’, ‘장애와 함께 나아가기’, ‘투석 끝의 죽음에 대한 불안’으로 나타났다. 셋째, 과정분석 결과, 연구참여자들은 4단계의 우울 과정과 4개의 우울 대처 유형으로 나타났다. 이에 본 연구결과를 토대로 혈액투석 중인 신장장애인의 우울 개입 및 완화를 위한 제언을 진행하였다.
Abstract
The objective of this study is to understand the process through which experiences with depression appear in renal impaired individuals receiving hemodialysis, what psychosocial phenomena and reactions they display in that process, and any influencing factors and situational contexts. In-depth interviews were conducted with 20 renal impaired patients undergoing hemodialysis who experienced subjective bouts of depression between the ages of 19 and 70. Their responses were then analyzed using grounded theory methodology. First, 128 concepts, 45 subcategories, and 17 categories were derived using open coding. Second, derived the results of the axial coding paradigm. Third, as a result of the process analysis, it was found that the study participants went through experiences with depression that could be broken down into four stages. Lastly, There were four types of mechanisms for coping with depression in renal impaired patients receiving hemodialysis. Therefore, based on th results of this study, policy suggestions were made for intervention and relief of depression in patients with kidney disease undergoing hemodialysis.
